The proposed electronic medical record design will contain patient demography which will record patient information; it will also contain the medial history of the patient. Another important part in the EHR is the presence of laboratory test information, images such as x ray images, payment records, physician information and the encounter type.

 The first step in constructing an entry relationship diagram is to identify all the parts in our electronic system, in our case the parts we will consider include the health representative, the patients, the encounter type, diagnosis and health disorders then we will consider the physicians.

 The second step is to determine the relationships between the entities we are to consider in our electronic system, in this case we will consider the first place where the information is first collected, we consider this as the entity health representative which will serve many patients, the patients will have many different complaints, these complaints will be referred to different physicians and therefore after the patient’s complaint we consider the relationship between the complaints and the encounter type and diagnosis. The encounter type will then determine the physician type.

 The final step before drawing our ERD is to analyze the relationship between the entities, when the relationships are determined we can now draw the diagram, we have to consider all the entities, the crow foot symbol will signify one entity serving too many of another entity.

 The following is the entry relationship diagram, the diagram contains all the entities we will consider and the joining lines show the type of relationship that exist between the entities, the diagram is as follows:

 The above diagram represents the entry relationship diagram, from the above it is clear that the health representative will serve many patients, the patient will have different complaints and therefore different encounter types. The encounter type will have different diagnosis or health disorder, finally the health disorders must be referred to a specialized physician and therefore one physician will serve on health disorder.

 The system will assist in recording and ease diagnosis of health disorders presented by consumers, when the patient visits the health center the health representative will first record the complaint and also names and other important information., the health representative will also try to find out whether this is the first time the patient is visiting or whether a regular patient, after this is established depending on the encounter type and previous health history the health representative will refer the patient to a specialized physician.

 The existence of a complex communication system the physician will also be in a position to refer to the stored complaint, also the health history of patient, allergies and other information. This access to information will make diagnosis easy and therefore improved services in a health care.

 Therefore we can conclude that this system will add value to the health center due to enhanced communication systems and also ease update and retrieval of information. The time taken to serve a patient will also be reduced due to increased efficiency in the health center.
